Service Hours Log
Service hours are due mid-May. Please fill in all parts of the Service Hours Log for each activity, especially the supervisor's signature and contact information. You may use the same sheet for multiple service opportunities. Please note that additional hours beyond the grade level requirement will not carry over into the next school year.

Grade 7: 10 hours
Grade 8: 10 hours
Grade 9: 10 hours
Grade 10: 20 hours
Grade 11: 30 hours
Grade 12: 40 hours
